Speculums are generally used to spread vaginally but can be used anally with great care. The Graves speculum is something of a standard for vaginal speculums. It works great for general opening, and is essential in a medical scene.
One knob sets the spread of the bills, the other adjusts the tilt of the bills relative to the speculum.
